What Happens After You Submit an Appointment Request

We will review the answers to the questions to ensure we can help.

We send a small text message to the number provided to verify you scheduled the appointment.

Once you respond, we confirm your appointment.

Intake Requirements.

Intake forms will be emailed to you.

Please check your spam folder if you have not received any emails within 24 hours.

You will be given a specific date to complete them.

If intake forms are not completed by the date, your appointment will be cancelled.

You must upload clear images of the front and back of your primary insurance card.

If you have secondary insurance, include that card as well.

You must upload a valid photo ID (typically a driver’s license).

Clients with commercial-only insurance must provide a credit card.
-
This card is used to process copays, deductibles, and coinsurance.

We are 100% telehealth.

No in-office appointments are available.
-
All forms are completed online.

All appointments are held over Zoom video conferencing.

All communication between appointments is handled through our secure app, Spruce.

You are not required to use Spruce, but it is the only method we use for communication outside of appointments.

Appointments cancelled less than 24 hours before the scheduled time will be charged.

No-shows will be charged.

Multiple no-shows may result in discharge from the practice.

You are responsible for notifying us immediately of any insurance changes.

If you have a large deductible insurance, payment may be required before the appointment.

Payment for Self-pay / Cash appointments will be charged up to 3 hours before the appointment.
