Please read through the information below, then complete the booking form at the bottom of the page.

⚠️Before You Book

Please confirm that we accept your insurance before continuing.

Please review our services. We provide Medication Management, not therapy.

If at any time you decide not to proceed, please notify us so we can cancel your appointment.

✅ What to Expect After Booking

Step 1:  We review your appointment request to ensure we can help

Step 2:  You'll receive a text message at the number provided to verify your request

Step 3:  Once you respond, we confirm your appointment and email your intake paperwork

Timeline:  Expect confirmation within 24 hours

 

📝 Intake Requirements

Complete Your Forms:

Intake forms will be emailed to you (check spam folder if not received within 24 hours)

You'll be given a specific deadline to complete them

⚠️ If forms aren't completed by the deadline, your appointment will be cancelled
 

Required Documents to Upload:

✓ Clear photos of your insurance card (front AND back)

✓ Secondary insurance card if applicable

✓ Valid photo ID (driver's license or state ID)

✓ Credit card on file (for copays, deductibles, and coinsurance)

💡 Tip: Make sure insurance card photos are clear and all information is readable

 

💻 How We Work

100% Telehealth Practice:

No in-office visits available

All forms completed online

All appointments via Zoom video conferencing

Credit card required to be on file

Auto charges to the card on file for copays, deductibles, and coinsurance
 

Communication:

Between appointments, we use Spruce (secure, HIPAA-compliant app)

Spruce is our only method for messaging outside of appointments

You'll receive download instructions after booking


 

⚠️ Important Policies

Cancellations & No-Shows:

Appointments cancelled with less than 24 hours notice will be charged

No-shows will be charged

Multiple no-shows may result in discharge from the practice
 

Payment & Insurance:

You must notify us immediately of any insurance changes

Large deductible plans may require payment before your appointment

Self-pay appointments: payment charged up to 3 hours before appointment
