Before continuing with scheduling an appointment,
please read the following.

  • Before You Book

    • Please confirm that we accept your insurance before continuing.

    • Please review our services. We provide Medication Management, not therapy.

    • If at any time you decide not to proceed, please notify us so we can cancel your appointment.

    • Booking form is at the bottom of this page.
       

  • What Happens After You Submit an Appointment Request

    • We will review the answers to the questions to ensure we can help.

    • We send a small text message to the number provided to verify you scheduled the appointment.

    • Once you respond, we confirm your appointment.

  • Intake Requirements.

    • Intake forms will be emailed to you.

      • Please check your spam folder if you have not received any emails within 24 hours.

    • You will be given a specific date to complete them.

    • If intake forms are not completed by the date, your appointment will be cancelled.

    • You must upload clear images of the front and back of your primary insurance card.

      • If you have secondary insurance, include that card as well.

    • You must upload a valid photo ID (typically a driver’s license).

    • Clients with commercial-only insurance must provide a credit card.

      • This card is used to process copays, deductibles, and coinsurance.
         

  • Important Information About Our Practice.

    • We are 100% telehealth.

    • No in-office appointments are available.

    • All forms are completed online.

    • All appointments are held over Zoom video conferencing.

    • All communication between appointments is handled through our secure app, Spruce.

      • You are not required to use Spruce, but it is the only method we use for communication outside of appointments.
         

  • Nomadic Behavioral Health: Key Policies

    • Appointments cancelled less than 24 hours before the scheduled time will be charged.

    • No-shows will be charged.

    • Multiple no-shows may result in discharge from the practice.

    • You are responsible for notifying us immediately of any insurance changes.

    • If you have a large deductible insurance, payment may be required before the appointment.

    • Payment for Self-pay / Cash appointments will be charged up to 3 hours before the appointment.
